New Patient Registration

If you have moved into the area and require a new GP then please call into the surgery to collect a form for each member of your family to complete or use the online registration form on our website.

New Patient Registration In Person

If you choose to register in person, you do not need proof of ID to register with a GP, but it might help if you have one or more of the following:

  • passport
  • birth certificate
  • HC2 certificate
  • rough sleepers’ identity badge
  • hostel or accommodation registration or mail forwarding letter

If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.

New Patient Appointments

If you are also registering for online access to your record – then ID is essential as follows:

Photographic ID
(must be in date)
Confirmation of address ID
(within the last 6 months only)
Current valid PassportUtility Bill – not mobile phone
Current Driving Licence (UK)Council Tax Bill
Bus PassHMRC document eg: P45/P60
Student CardNon-photographic driving licence
Biometric Residence PermitMortgage/Bank Statement
