If you have been sick and self-certificated for the first 7 days but still require to be off from your employment, please contact the surgery to book an appointment with the GP to discuss.
If you are enquiring regarding an extension to your previous medical certificate, please email the surgery – borstalvillagesurgery@nhs.net or drop into the surgery to complete a request form. Please state your initial length of time required and the GP will review your request. If the GP would like you to be reviewed you will be contacted by reception for this to be arranged.
